

The TIQUE PCP project to monitor and prevent complications with AI in heart failure begins its testing phase with patients
BeatBeat by Plat4TIQUE is a comprehensive platform for the treatment of patients with heart failure (HF), which improves the coordination of clinical teams, optimises care pathways and reduces hospitalisations through remote monitoring.
In this last phase, whose main objective is the implementation of the BeatBeat solution, the design has been adapted to the technical needs of the healthcare professionals of the three participating hospitals (Hospital de Sant Pau in Barcelona, Sant'Ottone Frangipane in the Italian region of Avellino and the Norrlands Universitetssjukhus in the Swedish region of Västerbotten) and ensure compliance with the specific requirements of each one, guaranteeing an efficient implementation and a customised experience for the needs of clinicians and the system. The pilot study has started at the Hospital de Sant Pau in Barcelona, where patients are being trained in the management of their signs and symptoms, healthy lifestyle habits and their interaction with the different levels of care.
“This type of programme allows a closer and more humane follow-up, which significantly improves the patient's experience and strengthens the bond with the clinical team”, say the healthcare professionals involved in the pilot.
Izertis, as the technological leader of the consortium, is implementing this artificial intelligence-based solution, with the support of Additum for the optimisation of the clinical process and Olidata as a specialist in emerging technologies, to prevent frequent decompensations in patients with heart failure. Through a decision support system, healthcare professionals will be able to anticipate and personalise care for each patient, improving the efficiency of the clinical process and the quality of care.
This project not only represents a significant advance in healthcare, but also drives the transformation of European healthcare systems towards a more efficient, integrated and patient-centred model.

BeatBeat by Plat4TIQUE has resulted in a solution that addresses the various challenges of managing and caring for HF patients:
- Define and standardise specific care pathways for HF patients throughout the care process.
- Improving transitions of care between cardiology, intermediate hospitals, primary care and home care.
- Coordinate multidisciplinary teams (MDCT) between the different levels of care.
- Remotely monitor HF patients in a holistic and reliable manner by integrating relevant data about the patient's condition.
- Facilitating patient self-management and learning, and capturing patient-reported results in a secure manner.
1- App for patients
An app (BeatBeat by Plat4TIQUE App) that not only monitors the patient's health status, but also provides them with a set of tools to empower them throughout their care journey. The BeatBeat App, together with the corresponding monitoring KIT, aims to support and empower patients to educate them in managing their signs and symptoms, healthy lifestyle habits and interaction with different levels of care.
The application collects passive and active data on the patient's clinical process and health status to share with MDCTs via the clinical tool. It serves as the main tool for understanding the patient's health status and clinical condition, as well as visualising the patient's care plan, along with educational and interactive material, to help the patient understand their disease and achieve a healthy lifestyle. It also has a conversational assistant (AI), co-designed with the clinical teams, to help patients use the app and answer some basic questions about their clinical condition.

2- Clinical Tool Platform for healthcare professionals
BeatBeat also has a clinician platform (BeatBeat by Plat4TIQUE Clinical Tool Platform) that displays key metrics related to the health status of HF patients, such as heart rate, weight and physical activity.
The platform incorporates alert prediction systems to anticipate and avoid decompensation, and suggests recommended actions based on the therapeutic objectives of each patient, thanks to Machine Learning (ML) models. This is always done under clinical supervision, promoting collaboration in the management of the patient's Share Care Plan, and the visualisation of these data. In this way, the ultimate goal is to optimise the entire care process for HF patients by aligning all involved parties (hospital and non-hospital) in the management and caring for the patient according to their needs and concerns.

Aiming to address bottlenecks and the loss of value resulting from misalignment, our approach seeks to optimise and increase efficiency across the care continuum, while improving quality of life and patient care, and reducing complications that can lead to hospitalisations.
3- Clinical dashboard
A clinical dashboard to transparently display results through continuous monitoring of patients' health status and the most important clinical data and KPIs of clinical procedures, serving as an objective focal point of process status. The clinical dashboards of the Heart Failure patient process show data, alarms and ongoing processes related to the patient, for fast and efficient action, aiding decision making and focusing on the management of healthcare resources, both within the hospital and between other entities in the care chain, such as home care or home hospitalisation.

Heart failure affects 62 million people worldwide and is the leading cause of hospitalisation in people over 65 years of age, placing a significant burden on health systems. With its commercial launch, the BeatBeat solution will transform healthcare delivery by offering a robust, adaptable and patient-centric platform that integrates seamlessly with existing healthcare systems, optimising resources and improving patients' quality of life.
